The standard for a favicon is 16px x 16px. Some web sites do not honor this
standard. <sigh>
Mostly websites that say they are standards compliant, but are too lazy to
validate their web sites or are using the Total Validator (FX Addon) or
cannot interpret the Total validator error and warning reports. Not that the
html validators can pick up this issue.

The current recommendation from MS is that web sites should provide both
16px^2 and 22px^2 favicon sizes within an "Icon Group' to accommodate both
90 and 120 DPI screen resolutions.
Here's a tweak that will help you understand what favicons are.
In IE type shell:sendto
This will open up your Sendto folder in Windows Explorer.
Right click on the file list (rhs pane) and select New>Shortcut.
Enter Notepad.exe, then Notepad. This will add the application notepad.exe
(text editor) to your send to context menu in Windows explorer.
Type shell:favorites in IE. This will open Windows Explorer at your
Favorites folder.
Right click on a file of type 'Internet Shortcut' and select Sendto>notepad
from the context menu. This will open the Guts of an 'internet shortcut' in
notepad.exe for you too look at. Notice the values for Icon and IconIndex.
Go back to IE and display the favorites pane. Right click on a 'Internet
Shortcut' file in your Favorites pane and select 'Properties', then the Web
Document tab, Change Icon button. You will be prompted to change the
'Internet Shortcut' favicon to a whatever your want. Save your changes.
Each time IE visits a site that is in your 'Favorites' it reads the meta tag
of that page that contains the Favicon address (by default =
webroot/favicon.ico) and updates the appropriate 'Internet Shortcut' file
(really just a text file with an ini format) to the web sites 'favicon' meta
tag value.
To prevent this over-riding your custom 'favicon' setting your must also
select 'Read Only' when you specify your own custom icon for the Internet
Shortcut.
