I am using Win XP.

When I save my scanned files to 'My Documents,' it defaults to
C:/DocumentsandSettings\User\MyDocuments and is 'duplicated' under 'My
Documents' in 'My Computer' as well.

However, if I save in a directory 'other' than 'My Documents,' will the
files only be listed once?

For some reason, the files are only duplicated when saved in My Documents.
Is the 'My Documents' folder under C:/DocsandSettings/User automatically
included with Win XP or was it specifically created for this program?

They are the same file.

The files are only saved in one location, which as you've discovered, is
C:/Documents and Settings\User\My Documents.

The links to "My Documents" that appear on the Start Menu, in the left
pane of the "My Computer" window, and as the icon for "User's Documents"
in the main pane of "My Computer" are only shortcuts.

If you save files in another directory, the "My Documents" links won't
point to that directory -- but you could make your own shortcut on your
desktop simply by right-clicking on the folder in which the files *are*
saved and selecting "Send To -> Desktop (create shortcut)".
