Add additional Word Document to legal Pleading

I need to add a Word document (Proof of Service) to a Legal Pleading. The
Court will not accept a Proof of Service that has the ruling and numbers.
How do I stop the Pleading templace so I can add a plain Word Document?

Not sure what you mean by "stop the Pleading template [sic]". Perhaps you
might want to post to a Word forum if you don't get an answer here, but you
may also be looking for someone with some knowledge of legal documents and
process.
